A 20-year-old student was taken to the emergency department of the hospital. He was in a semi-conscious state. He was having a severe headache, a purpuric rash on his arms and legs, and bleeding from his gums. He had been feeling ill for the previous 2-3 days. History He was taking anti-histamine medications for hay fever. Examination Body temperature was 39.4°C Heart rate of 100 beats per minute Blood pressure of 95/60 mm Hg Respiratory rate of 16 breaths per minute 95% O2 saturation on air The patient was following commands irregularly.

The Glasgow Coma Scale (GCS) was found to be 12/15 (eyes 3, verbal 4, motor 5). Neurologic examination showed neck stiffness.
