Rupture of the Achilles tendon can present several complications, one of which is suture granuloma. This tissue reaction against the suture material may represent a foreign body reaction, an end-stage response of the inflammatory wound-healing process to biomaterials. Suture granulomas can occur with both absorbable and non-absorbable suture materials, including silk as well. The following case presents delayed suture granuloma 30 years after open repair of the Achilles tendon with silk sutures.
