Sympathetic ophthalmia (SO) remains an uncommon but meaningful long-term consequence of open globe injuries, with incidence rising from 0.13% at six months to 0.18% at ten years , and most cases appearing within the first six months. In this 63,763-patient cohort, early eye removal (within 30 days) did not show a statistically significant reduction in SO risk. Importantly, individuals of Native Hawaiian or Other Pacific Islander and Hispanic ethnicity demonstrated significantly elevated hazards, underscoring the need for heightened monitoring in these groups. These results emphasize the importance of structured long-term surveillance and demographic-aware follow-up strategies after ocular trauma.
