Contemporary research and recent guideline updates highlight the need for early complication-focused therapy, continuous risk evaluation, and integrated support for mental health and social determinants of health. This article adapts these insights into a practical life course algorithm for primary care physicians, grounded in evidence published in the last five years [1–9].
