48Yr female having progressive increase in ascites, normal appetite , normal LFT, KFT, CBC. Non diabitic, non hypertensive. CT Abdomen shows B/L Mild to Modrate pleural effusion, Gross ascites, multiple subcentimetric & enlarged lymphnode seen in the mesentry & left para aortic location with largest one seen in tha left para aortic location, measuring 1.3cm in maximum short axis dimension. patient has no h/o fever and weight loss and cough expectoration. mantoux test revel 16 x 17mm.
pt has normal JVP & no pedal odema. Ascitic tap revels ADA- 9.6, Sugar 216, protien 4.05, clear yellow no clot, 340 cells lymphocytes mesothelial cells. serum protien 6.4, alb.3.1 serum albumin - ascitic fluid albumin = - .95 pt was taking DOTS treatment for last 1 month with no benefit. pt has some clinically significant nodes in left inguinal area which may be removed for histopathology, we also plan to…
