A  young man of 16 years, quite obese, weighing about 130 kgs, developed severe abdominal pain on the right side of the abdomen, was seen by the family physician, who prescribed him analgesics and antibiotics. The pain subsided after a few hours, but 24 hours later he complained of heaviness in the scrotum but no attention was made to this. The pain increased in severity and he was brought to me for consultation after a week. On examination, I did not find any abnormality in the abdomen but as soon as I stripped his genitalia, I found that his right testis was swollen, firm and very tender.
