The surgery namely TAPP (transabdominal preperitoneal) is used for inguinal hernia repair which is less painful, cost effective and helps the patient to go back to normalcy early. It involves the usage of mesh that is sutured to the anterior abdominal wall overlapping the defect widely. Dissection is done before the placement of mesh. The hernia sac is reduced and wide dissection is done in the preperitoneal area taking care while dissecting the triangle of doom formed by Vas and gonadal vessels medially and laterally.

Then the mesh is introduced. Since hernia is a defect of anterior abdominal wall, the placement of mesh needs to be done there and later it is sutured without causing injury to the muscles. The entire procedure of lap hernia repair without use of tacks is shown in the above video in detail. What is your experience of performing TAPP without tacks?
