A 63-year-old healthy man from rural area presented with a 1-month history of ulcerated paronychia, with a lesion at the base of the right thumb (as shown in the figure). He was asymptomatic and reported no pain. Examination A close physical examination revealed seven crusty, ulcerated skin lesions on the limbs (as shown in the 2nd image). As the ulcerations and lesions on the limbs were not consistent with chronic paronychia, a skin smear was obtained.
