Vagotomy - may be a solution for Unacceptably high 2 year failure rates (UPTO 80%) of Gastric Sleeve Surgery (in both BARIATRIC and AntiDiabetic Surgery). Though done extensively and strongly supported by International literature - Gastric Sleeve Surgery has high unacceptable failure rates in India. Reasons: 1) Receptive relaxation of stomach (due to vagus) in response to food 2) Tight pyloric sphincter- leading to excessive prolonged gastric wall tension 3) Ghrelin levels return to normal levels in 90% of patients- with in 1 year of surgery 4) Though Gastric acid secretion gets reduced after surgery that is sufficient to irritate the left over stomach .
