There is a general perception that vascular supply should be used when greater than 6 cm pieces of bones are grafted. This is the case for long-bone and large-bone joint reconstruction. However, the scientific evidence and recommendations are not clear. This article reviews whether there is a strong evidence which supports the importance of the need of vascularization for longer grafts.
Autologous bone grafts can be used as the mechanical structure to reconstruct or restore anesthetics and function. However, the decision whether to vascularize a graft when used for long -bone or large cm joint reconstruction requires analysis of benefits and risks.
