A 34-year-old woman, previously diagnosed with migraine, hypertension, and hyperthyroidism, was presented to the hospital complaining of complete visual loss. Medical history The patient was on levothyroxine, methyclothiazide, and triamterene. A week before the presentation, she was prescribed topiramate (50 mg). Later, her dose was increased to 100 mg of topiramate. Examination Visual acuity: The best-corrected visual acuity came out to be in counting fingers. Anterior segment examination: The investigation revealed bilateral conjunctival chemosis, corneal edema, and a shallow anterior chamber.
Gonioscopy: The scan showed closed-eye angles. Intraocular pressures (IOP): The pressure was elevated in both eyes, i.e., 49 mm Hg in the right eye and 51 mm Hg in the left eye. B-Scan ultrasound: Peripheral choroidal effusion was found in both eyes. Figure 1. Slit-lamp photograph at…
