As we are get filled -up the LAMA Consent Form (Leave against the Medical Advise) for the Hospital safety for those patient who deny to stay in the hospital and insist to doctors for the discharge of the patient without knowing the risk of the patients. But those patients wants to care there self they request for the DOPR (Discharge on patient Request), in this case we again get filled-up the LAMA consent form for the hospital and doctor safety. who we can use the DOPR consent form and what kind legal language we can use?
