Case Presentation A previously healthy prepubescent boy presented to the pediatric surgery department with worsening pain and deterioration of an old injury on his right great toe. Medical History Three months earlier, the child had undergone nail removal for persistent onychomycosis (Cover Image A). He was initially discharged with topical antibiotics, but due to ongoing inflammation, was later prescribed a 10-day course of oral cefprozil.
About three weeks after the procedure, a new trauma to the same toe led to the development of granulomatous tissue (Cover Image B). Surgical debridement was performed a second time, and he was again discharged on oral cefprozil.
