Clinicians come across patients from varied backgrounds and beliefs. Nowadays because of internet a lot of medical information is available and sometimes patients come with their own analysis and diagnosis about their own symptoms. And if your and their diagnosis doesn't match they raise doubt and start (counter-)questioning. On the other hand, you also come across uneducated patients, who have no scientific knowledge and understanding of their symptoms.
So patient's care and treatment follow-up may become an issue. In short, both types of patients are reluctant to follow your advice. In your opinion, who do you find difficult to convince the 'over-informed patients' or 'under-educated patients'?
