I work in a PSU hospital right now. To my utter surprise there has been an ongoing wrong practice of no perioperative notes being written by anaesthetist for reasons best known to them. Inspite of my raising this issue repeatedly with management there is no change in the practice.I want to know Who owes the responsibility for missing perioperative notes? Can surgeon in anyway later held responsible for this omission done purposefully by anaesthetist?

How can surgeon safeguard his interest to avoid litigation on this ground later? What possible action is possible against erring anesthetist and management and which legal body can take that? Can anaethesia related consent be included in surgical consent or it needs to be a separate one and tobe explained and signed by anaesthetist?…
