Case presentation A 52-year-old woman comes to the hospital complaining of fatigue. A complete blood count test reveals anemia. The hemoglobin level was noted to be 8.1, and a hematocrit of 23.9. A rectal exam comes out to be guaiac positive for blood.
Past medical history Myocardial infarction She has a long history of rheumatoid arthritis and has joint deformities in both her hands A gangrenous fifth digit of her left leg Multiple deep venous thromboses of the lower edges Possible systemic lupus erythematosus (constituting a possible mixed connective tissue disorder) Adrenal gland insufficiency (borderline) Medication history The patient is on multiple medications for joint disease, including coumadin and hydrocortisone (100 mg every 8 hours) .
