A 15-year-old boy presented to the emergency department with vomiting and epigastric pain appearing in the back. The onset of pain was gradual and lasting from 3 days. Earlier, he didn't face any of these pain episodes. The patient was sweaty and tensed. He says he was unable to lie flat for the examination and vomits persistently.

The abdomen was not distended and he had normal bowel sounds. Rectal examination was unremarkable.
