Case presentation A 60-year-old female reported to the emergency department with lower gastrointestinal bleeding. Medical history The patient was a smoker, suffering from Crohn’s disease and antiphospholipid syndrome. The patient was under medication (warfarin and mercaptopurine) for antiphospholipid syndrome. Hematological findings Pancytopenia No myelodysplastic syndrome Bone marrow biopsy A bone marrow biopsy was considered unsafe since the patient had a fever and desaturation episodes.
Thoracic CT scan Mild bilateral pleural effusion, with no signs of pneumonia or lymphadenopathy. The patient died due to respiratory insufficiency and severe lactic acidosis ten days after hospitalization.
