This case describes a novel approach to treat severe odontogenic maxillary sinusitis (OMS) spreading to the frontal sinus. 1 The approach includes conservative maxillary sinus management (CMSM) without maxillary sinus mucosa (MSM) removal. Case presentation A 54-year-old man came in with the chief complaint of pain radiating from his right cheek to his forehead. The diagnosis of OMS caused by teeth #25 and #26 was confirmed. His symptoms did not improve with endodontic treatment and long-term antibiotic administration.
