A 47-year-old man is referred to a psychiatrist at his employment assistance program because of continuing conflicts on the job. This is the third time the patient has been referred to a psychiatrist under just such circumstances. Patient History: He lost two previous jobs because of conflicts with coworkers. The patient states that people do not like him and would like to see him fail.

He cites as an example one instance in which one of his colleagues was late in sending him some material he needed, resulting in the patient being unable to complete his assignment in a timely fashion. Although the colleague apologized for the mistake, the patient says that he knows that this man is “out to get me fired.” He has since broken off all contact with this coworker and refuses to speak with him directly, preferring to use only written communication.
