Case presentation: A 65-year-old woman with a history of end-stage renal disease was admitted to the hospital 19 months post-renal transplant with complaints of cough, fever, and shortness of breath. Laboratory examination: Complete blood count (CBC) White blood cells: 1.41 K/mL Hemoglobin: 12.9 g/dL Mean corpuscular volume: 90 fL Platelets: 163 K/mL Histological examination was negative for other infections except for COVID-19. Confirmatory RT-PCR test was positive for SARS-CoV-2. The blood smear showed characteristic inclusions in the form of dots and spots in the neutrophils (cover image).
