A 69 year-old-male presented to a diabetes specialty clinic with complaints of suboptimal diabetes control, weight gain and foot pain. History of presenting illness The patient had a 5-year history of type 2 diabetes mellitus (T2DM) and his HbA1C was greater than 8% for the past several years. Prior to the diagnosis with T2DM, the patient was in a prediabetic stage for 2 years with average fasting glucose ranging from 118 to 127 mg/dl. In addition, the patient had a history of weight gain and was advised to lose at least 4-5 kgs.

Medication History The patient was on glyburide medication, 2.5 mg; however, due to complaints of sudden episodes of dizziness, sweating, and mild agitation, he discontinued the therapy.
