An alleged medication error at Banda Civil Hospital in Madhya Pradesh's Sagar district has prompted an official investigation after a 19 month old child reportedly suffered permanent vision loss. The child was brought to the hospital with complaints of cough, cold, and eye redness. Following evaluation in the outpatient department, the attending pediatrician prescribed symptomatic treatment that reportedly included eye drops, paracetamol syrup, an injection, and medication intended to help clear respiratory secretions.

According to the family, the medication prescribed for phlegm relief was mistakenly instilled into the child's eyes by hospital staff instead of the prescribed ophthalmic drops.
