A 75-year-old man was brought to the hospital with an episode of dizziness. The patient felt unwell even when seen 30 min after the onset. Case History He was well until 6 months ago and then started having falls. On some occasions, the falls were associated with loss of consciousness, although unsure of the length of time he was unconscious. On other occasions, he has felt dizzy and has had to sit down, but not lost consciousness.

These episodes usually happen on exertion, but once or twice they occur while sitting down. He recovers over 10–15 min after each episode. He lives alone, and most of the episodes have not been witnessed. Once, a visitor was accompanying when he blacked out and based on his pale looks and stillness, the visitor assumed him died.
