Case presentation A 66-year-old male patient presented to the Emergency Department (ED) of a hospital with a 3-day history of cough, congestion, and pain in his chest and left abdomen. Several months prior to the presentation at ED, the patient had immigrated to a different country after staying in a refugee camp for a period of 20 years. Physical examination The patient’s heart sounds were best heard on the right side of the chest. On palpation, tenderness was observed in the costochondral joints and the left upper quadrant.
