Antibiotic therapy is considered an effective and safe alternative for treating uncomplicated acute appendicitis in adults; however, long-term randomized clinical trial data beyond 5 years are limited. To address this gap, a study evaluated the 10-year recurrence of appendicitis and appendectomy rates in 257 patients with uncomplicated appendicitis treated with antibiotics. a median follow-up for 10 years, the study found that appendicitis recurrence occurred in 37.8% of patients treated with antibiotics, with a cumulative appendectomy rate of 44.3%.
Complication rates were significantly lower in the antibiotic group compared to appendectomy (8.5% vs 27.4%), while quality of life was similar between groups (median health index value, 1.0 in both).
