Case presentation A woman presented to the emergency department (ED) with breathing difficulties due to tightness in her chest that lasted for 30 minutes. The patient had a 5-year history of persistent bronchial asthma. She also experienced an aggravation of the illness a year ago. Physical examination BP:  80/50 mmHg Pulse rate: 122 bpm Chest sound : Presence of polyphonic rhonchi and scattered bilateral crepitations. ECG examination Elevated ST segment at leads V1 – V3.

The patient had a nterior and septal wall hypokinesia with an ejection fraction of 30%. During the patient's hospital stay, her troponin levels fluctuated. She was on a 2-week ventilator support in the ICU and was later discharged with a prescription for regular medication.
