Death in the OR is the most undesirable experience for any doctor. Most patients are managed well because it is already clear that they are sick. Utmost attention is given to them. Despite any explained consent, a cardiac arrest is most undesirable. Then there those freak cases with unexpected outcomes. Sometimes, it may be a hypersensitivity to local anesthesia or wrong guesses. Anything can happen in practice.
It is better to ensure everything is available in every case. I have the anesthetist colleague who even knows about the local case even though they may not be there. I think many of us would have experienced or heard of such cases. I am also sure that the OR is the best place to manage serious problems. Most unacceptable in today's practice but truth pays. What should be the approach of doctors in such death cases?
