In hemiplegia, one of the important signs elicited in the clinical examination is clonus- ankle and patellar. But we are not coming across clonus in all the hemiplegia cases. What is the reason for this difference in the presentation? made me think in 1997. Since the CT Scan was just introduced in our place at that time, I got the answer out of the CT Brain findings.

Whenever affection of basal ganglia was reported along with internal capsule affection in CT Scan Brain, I have made out Clonus especially Ankle clonus and at times Patellar clonus.
