Case presentation A 5-day-old infant was brought to the emergency department with the chief complaint of 102˚F fever along with progressively poor oral intake and no wet diapers in the last 12 hours. Medical history Remote history of herpes simplex virus (HSV) Mother was compliant with prophylactic acyclovir. Clinical presentation Sepsis with high fever Ill-appearance, seizures or other focal neurologic signs, thrombocytopenia, mucocutaneous vesicles, elevated liver transaminases, and cerebral spinal fluid (CSF) pleocytosis Investigations Sepsis evaluation was carried out.
Blood, urine, and cerebral spinal fluid (were collected for both culture and HSV polymerase chain reaction testing.
