Case presentation A 30-year-old teacher presented with a persistent cough with yellow sputum, fever, episodes of night sweats, and pleuritic right scapular pain. He had no other medical history and denied breathing problems, hemoptysis, or diarrhea. He had recently traveled to several countries like Japan, Thailand, Cambodia, Israel, and Laos but had taken recommended vaccines. He uses a single-room humidifier every night while sleeping.

He had no history of recurrent infections and denied smoking or drinking alcohol regularly.
