A 67-year old woman received a living donor renal transplant. History She had been on continuous ambulatory peritoneal dialysis (CAPD) for 9 years. No history of peritonitis during CAPD Graft function was good. Serum creatinine level, 5 days after transplantation– 0.9 mg/dl The patient received combination immunosuppressive therapy with methylprednisolone, tacrolimus, mycophenolate mofetil and basiliximab. Examination 8 days after transplantation, the perirenal fluid collection was detected during routine USG (Figure a).
The fluid collection increased daily and hence on the 14 th day, simple aspiration was performed to relieve the lower abdominal pressure. The creatinine level of drainage was equivalent to that of serum. However, after aspiration, the fluid immediately reaccumulated in the perirenal space (Figure b).
