Pregnancy at present 28 weeks with previous LSCS spotting at times and some time fresh blood at times, brown discharge on Usg central placenta. Previous covering  is with thick vascular sinuses I feel chances of accreta. I am scared.

What best to be done to save the mother? Should I try to remove the placenta or just tie cord at placental end and later put her on methotrexate or try total hysterectomy without removing placenta in any way to save mother?
