An 87-year old female suffering from severe symptomatic aortic stenosis (mean gradient 55 mmHg, aortic valve area 0.85 cm 2 ), underwent transcatheter aortic valve implantation (TAVI). Six months later, she presented with ischemic stroke with convulsive seizures and intermittent fever. Transoesophageal echocardiogram (TOE) showed large mobile vegetation measuring 1.3 × 0.6 cm attached to transcatheter prosthetic valve struts and an abnormality of the aortic root resembling a mass extending from the aortic root to the left atrium. Blood tests revealed thrombocytopenia and elevated inflammatory markers. Blood cultures were positive for Corynebacterium spp.
