Case presentation A woman in her 50s presented with a sudden onset of right lower quadrant abdominal pain with vomiting and melena. The patient had a history of the polycystic ovarian syndrome, diabetes, stroke, and coronary artery disease. She had undergone coronary artery bypass graft surgery six years ago. On physical examination, the abdomen was found to be tender to palpation with rebound and voluntary guarding.
