A 70-year-old male presented with painful enlargement of a goiter on the left side for one month and complained of difficulty in breathing with intermittent dysphagia. He did not have any symptoms of local infiltration and was clinically euthyroid. Medical history : Well-controlled type 2 diabetes mellitus and goiter for 30 years Physical examination: A hard mass measuring 8βcmβΓβ7βcm was observed from the left thyroid lobe with gross tracheal deviation to the right side. Also, two mobile lumps anterior to the mass in the subcutaneous tissue plane were observed.
The right thyroid lobe had multiple palpable nodules and was moderately enlarged. There was no cervical lymphadenopathy. Laboratory examination: Thyroid-stimulating hormone (TSH) and free thyroxine (T4) levels were within normal limits.
