Case presentation A 52-year-old male presented with a month-long history of nonradiating right thigh pain, swelling, and erythema. Recently diagnosed with diabetes, hypertension, and ischemic cardiomyopathy, his severe pain rendered him disabled, requiring assistance to walk or use a wheelchair. Despite multiple emergency and urgent care visits, oral antibiotics prescribed for presumed cellulitis provided no relief. He had no history of fever, trauma, or intravenous drug use.
