A 40-year-old male presented to the medical center with symptoms of an altered mental state. He was suffering from neurosarcoidosis. Primary examination showed that the patient was lethargic but responded to verbal stimuli. He was hypothermic (32°C) and hypernatremic (176 mEq/L). His physical examination was otherwise unremarkable. The patient was admitted to the ICU, and fluid management along with intranasal desmopressin administration was done.

He was externally warmed using blankets, and his mental state improved on the improvement of serum sodium.
