A patient of mine, young girl of 20 years came to me with symptoms of UTI for 10 days. On close questioning, she revealed symptoms suggestive of Vulvo-vaginitis. I immediately asked for urine routine and to my surprise, she had ++++ sugar. The I asked for complete set of investigations, BSF 230 mg%, PP 340 mg% and HbA1c of 10.2%.

I treated the case as Type 2 diabetes and gave her Metformin 500mg tid, Glibenclamide 5mg tid, ( since ours is a free clinic and the pat.cant afford, I didnot give her other drugs). After 4 weeks the sugar level has come down to 170 and 230, but still high only. I have gone for Type 2 diabetes treatment inspite of suggestions from colleagues to treat it as type 1 because  i thought that we can give the treatment a try and secondly, the young patient wont comply with injection treatment.
