diagnostically challenging case needs diagnosis

A female of 30 years old rural background, involved in animal husbandry, presented with cough and breathlesness of one month. No fever, no other systemic symptoms.HR 110, BP normal, no cyanosis or clubbing.RS -crackles infrascapularly on both sides ,other systems NAD. Spo2 86%,TC 12k,ANA , HIV Neg,Sputum AFB 3samples neg, RBS, U/cr.N,cxr-reticulonodular pattern with cavities, no hilar adenopathy. HRCT reticulonodular pattern with cavities.

ANCA negative. 10 th day shecould not maintain spo2 and put on NIV. Bronchoscopy with lavage is planned.Echo shows Severe PH of 74mm of Hg.
