A sputum positive case of TB will harbour the infection for a long time before becoming sputum positive. A slow progression with weight loss evening rise of temperature, cough and radiological signs take the time to develop. The presence of TB bacilli is becoming mandatory before the ATT or even a high ESR. A short visit to the doctor and a short course of antibiotics may temporarily stunt the TB bacilli before the actual symptoms surface. When should the ATT be started and what are the options to start ATT to reduce sputum positive TB?

Extrapulmonary TB is dangerous to the patient but not to the society. So a higher incidence of Extra TB may be welcome than Pulmonary. I may be wrong. But safe for society. Give your opinion.
