Granulomatous hypophysitis is a rare inflammatory condition that mimics pituitary adenoma. Diagnosis of this condition is challenging and is confirmed by histopathology examination. The outcome of the transsphenoidal approach is favorable in confirming the diagnosis and relieving symptoms related to mass effect. History of presenting illness  A 37‐year‐old female, who was otherwise healthy, was presented with a history of 2 episodes of vomiting, and gradual onset of headache for 1 week which worsened 1 day prior to admission.
