Case presentation A primary care practitioner referred a 19-year-old male patient with painful tongue lesions to an otolaryngologist. The patient reported having these tongue lesions for over three months, and the pain worsened with spicy food consumption. Clinical examination Bilateral, elevated, slightly raised leukoplakia on the tongue Tender and non-scrapable lesions No lymph node enlargement, weight loss, or digestive upset Absence of fever, fatigue, or rash Medical history No significant medical history Clotrimazole lozenges for the lesions Mild alcohol consumption History of engaging in oral and anal intercourse   Histopathological investigation A biopsy of the right tongue lesion was performed.
