Ascending aortic hemiarch reconstruction, a lower-complexity surgical approach, may provide comparable long-term outcomes to extended arch reconstruction in patients aged 65 years and older with acute type A aortic dissection (ATAAD). To evaluate this, a risk-adjusted analysis using the STS Adult Cardiac Surgery Database linked to Medicare data examined long-term survival and aortic reintervention rates following hemiarch versus extended arch procedures in 3,562 ATAAD patients. The study found that ascending aortic hemiarch reconstruction and extended arch reconstruction resulted in similar long-term survival and rates of aortic reintervention in patients aged 65 years and older with acute type A aortic dissection.
