Case presentation A 28-year-old female presented with epigastric pain that persisted for the past two months. The pain gradually increased when she sat for a longer time and mitigated while lying down. She had the same pain six years earlier, and a mesenteric cyst was identified. Ultrasonography-guided aspiration was performed at that time.

Initial examination Cytology analysis – Inflammatory cells with high numbers of macrophages and absence of neoplastic cells Vitals were normal. Abdominal examination – Soft elastic abdomen and soft cystic fluctuant mass (18 x 15 cm) palpable in left hypochondriac, epigastric, and umbilical area Abdomen ultrasonography – Heterogenous hypoechoic cystic lesion (19.8 x 15.6 x 18 cm) with minimal vascularity, echogenic septae, and debris in the pancreatic tail.
