Case presentation A 68-year-old male was admitted to the hospital with worsening shortness of breath and continuous weight loss over the past six weeks. In addition, the patient admitted to smoking 16 packs of cigarettes a year. Examination Abdominal and thoracic CT— 30 mm solid lesion with bilateral pleural effusion (Figure 1) An 18 FDG-PET-CT scan was carried out. This revealed normal metabolic activity in the pleura, mediastinal lymph nodes, and tumor (Figure 2).

Mucosal biopsy, bronchoscopy, and bronchial lavage all returned normal results.
