A 58-year-old healthy female patient 5 feet 2 inches posted for left knee replacement was given spinal Anaesthesia with 2.9 ml Bupivacaine heavy. No Adjuvant. Epidural catheterisation was done — that too in insufficient dose infusion 2 ml per hour 20 ml 0.125% Bupivacaine + fentanyl 100 1amp. A patient was weeping with pain immediately after surgery. They gave tramadol, by a senior so-called most experienced cardiac anaesthetist. He gives a name as safe Anaesthesia to this. Is this Really safe? Is this Adequate?
Often I see this type of Anaesthesia— and they name it safe Anaesthesia. 2.9 ml without Adjuvant — what are you going to achieve? Only the agony of the patient. Give opioids / Dexmedetomidine as Adjuvant liberally. Your over-exaggerated fear should not lead to inadequate Anaesthesia. Even, in general, I observe the same. Share your opinion.
