A 55-year old female patient presented with loose stool, abdominal pain and occasional vomiting. O/e patient was cons/ori BP 70/50 pulse :110/min fingerprick random blood glucose 450mg/Dl. She is a case of DM2 on OHAs; no other comorbid.
The patient was stabilized with several lts of isotonic saline BP :120/74 p:89/min blood sugar 233mg/Dl, insulin was not used yet for initial lowering of blood sugar; she improved a bit but lethargic and anorexic. What should be the next step? Kindly suggest the cause of abrupt deterioration of her condition and management.
