Blocking sciatic nerve for analgesia of the lower extremity is a challenge for an anesthesiologist. A recent study describes a new method using ischial tuberosity as a landmark which needed fewer needle pass than conventional method involving ultrasound aided subgluteal approach with patients in lateral Sim’s position. Blockade of the sciatic nerve is necessary for complete analgesia of the lower extremity using peripheral nerve blocks. The single most important aspect in the practice of regional anesthesia is the skill and experience of the clinician performing the anesthesia.
