A 45-year-old man attended for a routine blood pressure check, but while in the waiting room he experienced severe central chest pain. The patient appeared pale and sweaty. An ambulance was called and arrived within 10 min, at which time he was still in pain. He is en route to the hospital. The patient is responsive, breathing spontaneously and has a BP of 120/50.
The ambulance inserted an intravenous cannula and applied oxygen via a mask and administered two doses of glyceryl trinitrate (GTN) sublingually. Past medical history This includes high BP, for which the patient takes the angiotensin-converting enzyme (ACE) inhibitor ramipril.
